INDEMNITY AND ACKNOWLEDGEMENT

I, the undersigned, being the parent/legal guardian of the swimmer named herein, alternatively the swimmer himself/herself where applicable, hereby acknowledge and agree that participation in swimming training, galas, competitions, camps, travel, fitness activities, and any other activities arranged by or associated with Cybersmart Aquatics involves inherent risks, including but not limited to physical injury, illness, property damage, and other unforeseen risks associated with sporting participation.

I voluntarily permit my child/myself to participate in such activities and hereby indemnify and hold harmless Cybersmart Aquatics, its owners, employees, coaches, agents, representatives, contractors, and affiliated persons from and against any loss, damage, injury, claim, cost, or liability arising from participation in any Cybersmart Aquatics activity or event, except where caused by gross negligence or wilful misconduct.

I further acknowledge and accept responsibility for ensuring that my child/myself is medically fit and able to participate in swimming and related activities.

I also acknowledge that swimmer participation, gala eligibility, and club representation are subject to compliance with Cybersmart Aquatics policies, including but not limited to membership status, payment obligations, registration requirements, and governing body rules.

I hereby confirm that I have read, understood, and agree to abide by the Cybersmart Aquatics Terms and Conditions, policies, rules, and guidelines.
